Private Duty Nurse (RN/LPN) – 1:1 Home Care
Job Summary
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.
Client Profile & Clinical Needs:
- Location: Holly
- Age: 3 month old male
- Diagnosis and Care Required: Born with congenital birth defects requiring high-level care to manage floppy, easily obstructed airways and safe feeding via alternative methods. He will require trach management as well as oxygen management as needed.
- Nutrition: via G-tube
Family & Home Environment:
- Primary Caregivers: Mom and Dad
- Household: 3-year-old sibling and a 1 friendly dog.
- Night Shift: 7:00 pm - 7:00 am
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
Schedule & Pay
- Full-time and part-time shifts available
- Day, night, and weekend shifts
- Weekly pay + optional daily pay

Qualifications
- Active RN or LPN license in the state of Michigan
- Home care or private duty experience preferred (not required)
- Trach/vent experience a plus (training provided)
- Strong communication and critical thinking skills
Patient Population
- Pediatrics (birth–21 years)
- Adults with complex medical needs
