Willow Creek Healthcare Center is seeking a compassionate, organized, and dependable Non-Licensed Case Manager to join our team. This position does not require a professional license.

The Case Manager works closely with residents, families, healthcare providers, insurance companies, and community resources to coordinate care from admission through discharge. The ideal candidate has strong communication and critical-thinking skills, thrives in a fast-paced healthcare environment, and is committed to providing an exceptional resident experience.

Essential Duties and Responsibilities

  • Communicate regularly with residents and families regarding plans of care, therapy services, treatment protocols, and discharge planning.
  • Attend daily morning meetings and stand-ups to support communication and coordination among departments.
  • Serve as a resource for residents and families by answering questions and helping them understand care plans and available services.
  • Monitor residents’ needs and coordinate updates to services as their conditions or circumstances change.
  • Organize and prioritize daily responsibilities for newly admitted, current, and discharging residents.
  • Oversee and coordinate admission and discharge activities.
  • Coordinate referrals to and from the facility.
  • Perform utilization review activities to support appropriate, timely, and cost-effective care.
  • Coordinate services with residents, families, healthcare providers, facility financial services, insurance companies, and third-party payers.
  • Maintain accurate documentation and complete required paperwork within established timeframes.
  • Communicate effectively with physicians, nurses, therapists, payers, ancillary providers, and other members of the interdisciplinary team.
  • Promote cooperation among residents, families, caregivers, and healthcare partners.
  • Protect residents’ privacy and maintain the confidentiality of all health information.
  • Develop organized work processes and adjust priorities in a busy environment with rapidly changing needs.
  • Maintain professional, respectful, and honest relationships with residents, families, staff, and community partners.
  • Perform additional duties as assigned.

Qualifications

  • No professional license is required.
  • Bachelor’s degree in social work, human services, healthcare administration, nursing, or a related field preferred.
  • Previous experience in healthcare, case management, admissions, discharge planning, social services, or skilled nursing strongly preferred.
  • Knowledge of Medicare, Medi-Cal, Medicaid, insurance benefits, and authorization processes preferred.
  • Experience with PointClickCare or another electronic medical record system preferred.
  • Certified Case Manager credential is a plus but not required.
  • Intermediate computer skills and the ability to learn facility systems.
  • Strong communication, organization, documentation, and time-management skills.
  • Ability to work independently, exercise sound judgment, and manage multiple priorities.
  • Compassionate and professional demeanor with a commitment to resident-centered care.

Skills and Abilities

  • Ability to read and understand technical procedures, policies, and operational manuals.
  • Ability to clearly present information and respond to questions from residents, families, staff, providers, and managers.
  • Ability to apply basic mathematical concepts, including fractions, percentages, ratios, and proportions.
  • Ability to solve practical problems and interpret written, verbal, diagrammed, or scheduled instructions.

Physical Requirements

This position is primarily sedentary and involves frequent sitting, computer use, and documentation. Regular walking throughout the facility is required to meet with residents, families, and staff. The employee may occasionally stand for meetings and care conferences and lift or carry items weighing up to 20 pounds.
