Job Title: Occupational Therapist - Discharge to Assess

Pay Rate: Up to £38 per hour

Job Description:

Liquid Personnel is seeking a skilled and compassionate Occupational Therapist to join a dynamic Discharge to Assess (D2A) team for its client in Slough.

This is a fantastic opportunity to make a real impact by supporting timely and safe hospital discharges, ensuring patients receive the right care in the right place.

You will:

• Support with Trusted Assessments as needed.
• Use your clinical expertise to screen and process hospital discharges.
• Facilitate smooth transitions from hospital to home or community settings.
• Work collaboratively with multidisciplinary teams to deliver person-centred care.

Requirements:

• HCPC-registered Occupational Therapist.
• Previous experience in hospital discharge or community rehab settings.
• Strong assessment and decision-making skills.
• Ability to work independently and as part of a team.
