Job Title: Adult Social Worker - Hospital Discharge  
Location: Barnsley (Hybrid)
Pay Rate: £35 per hour

Liquid Personnel is currently recruiting for an Adult Hospital Social Worker on behalf of our client. This is an exciting opportunity to join a dedicated hospital discharge team, where you will play a vital role in supporting adults during their transition from hospital to home or other care settings.

Responsibilities : 

• Experience working with health partners or hospital social work discharge teams.
• Strong social work values that guide professional practice.
• Focused on helping individuals achieve the best outcomes following hospital discharge.
• Committed to promoting independence through a strengths-based approach.
• Collaborates effectively with individuals, families, carers, and communities.
• Supports informed decision-making processes.
• Works closely with health colleagues under the hospital’s Discharge to Assess process.

Qualifications & Experience:

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
