Responsibilities
Otterbein is seeking an enthusiastic and professional Hospice RN Case Manager
- This role is responsible for primary caseload.
- This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.
Qualifications
- Education: Bachelor’s degree in nursing preferred. Graduate of approved nursing program
- Licensure/Certification: Valid Ohio Registered Nurse (RN) license
- Experience: A minimum of 2 – 4 years’ direct patient care experience in hospice and Case Management experience preferred
- Experience: Experience with end-of-life diagnosis preferred

- Provides personal care
- With respect to patient rights, ensures all care is provided with respect and dignity to patients and their families
- Demonstrates and maintains a positive, professional manner
- Keeps a record of hospice STNA plan of care for each patient assigned
- Observes, records, and reports the patient’s care, symptoms, emotional and physical status
- Watches, observes, and reports any physical and gross behavioral changes in the patient’s condition or family situation to the RN
- Assists in feeding the patient as needed
- Communicates appropriately with the patient/family and agency personnel
- Attends required in-services and meetings as identified by the agency (must have twelve (12) hours of in-services a year).
- Attends and participates in regularly scheduled Hospice interdisciplinary team (IDT) meetings to coordinate plan of care, follow up on changes and other communication, problem-solve, receive staff support and in-service education
- Participates in Quality Assurance/Performance Improvement (QAPI) activities, as indicated
- Will need to travel to see patients between Lebanon, Middletown, and Springboro
Qualifications
- Minimum age requirement of 18.
- Education: Must have completed the 9th grade, with ability to read or write at an adequate level. High school graduates and GED preferred.
- Certification: CNA/State Tested Nursing Assistant (STNA) certification - Must be on the State Tested Registry in Ohio and remain active, CPR required.
- Experience:
- Minimum of one (1) year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.
- Must demonstrate the ability to be able to work within a multidisciplinary team setting.
- Understanding of Hospice philosophy and needs of terminally ill.
- Valid driver’s license and reliable transportation that is insured in accordance with Ohio requirements.

Hospice Care Navigator

Overview
Otterbein is a not-for-profit, faith-based Hospice that is a national leader and innovator in elderly care services.
The Hospice Care Navigator is responsible for developing, maintaining and executing a Marketing and Sales Plan in support of the Otterbein Hospice group and growing census. This includes managing all activities associated with the sales process from the initial connection with a referral contact, and/or their families, through informationals and ultimately enrollment in hospice care.
Responsibilities
- Develop and manage the contact and leads database.
- Grow Census to exceed budget.
- Establish relationships with key referral contacts (doctors, nurses, social workers, ministers, hospital administrators, senior center liaisons, etc.).
- Make outbound calls to drive on campus or on location appointments, conduct appointments to complete the process; and represent Otterbein at various public functions.
- Develop, maintain, and execute a detailed “Marketing and Sales Plan” – inclusive of programs to improve sales, internal and external events and programs, etc. Track and report on the source of referrals and build the sales plan to reflect this distribution and add new referral sources.
- Educate the public, Otterbein partners, and potential and actual referral sources on end of life, advance directives and hospice eligibility and hospice care.
Qualifications
- B.S. in Health Care, Social Work, or Marketing or Business Management preferred, or equivalent experience.
- Valid state of Ohio RN or LPN license
- Licensure/Certification: Ohio Driver’s license
- 2+ years of sales/referral marketing experience preferably in hospice.
- Must have strong interpersonal skills and possess excellent communication skills (listening, writing and speaking).
- Must be able to communicate with older persons (and associated families) who may be ill, disabled and/or emotional.
- Must be detail oriented, able to prioritize and utilize effective time management skills and be flexible to adjust to impromptu situations on a regular basis.

The Hospice Social Worker is responsible for the provision of a wide range of psychosocial services to terminally ill patients and their families.
Shifts Available: PT
Responsibilities:
- Conduct initial comprehensive psychosocial and bereavement risk assessment of patient/family/caregiver in a thorough, accurate, and compassionate fashion within five (5) calendar days after the patient’s election of Hospice care.
- Function as an expert and facilitator at obtaining and making referrals to community resources to meet the needs of patients and families including obtaining financial assistance when indicated and referring patients and families to community agencies with appropriate follow-up.
- Assist family/caregiver in placement of patient in a facility for basic care, respite or inpatient when appropriate as indicated.
- Provide psychosocial and anticipatory grief counseling to patients, families, caregivers and significant others relative to the impact and implications of the terminal disease and patient care.
- Assesses the psychosocial status of patient/family as it relates to capacity to cope with diseases, terminal illness, environment and death. Provides counseling services as applicable and assists in utilizing community resources. Communicates findings to the team.
- Participate in the development of the plan of care and treatment. Holds case conferences as needed.
- Prepare social histories, evaluations and plan interventions based on findings.
- Updates, maintains and submits clinical documentation according to agency guidelines in an accurate and timely manner. Assists with discharge planning.
- Attends and participates in scheduled interdisciplinary team meetings (IDT) to coordinate care plans, follow up on changes, problem solve, identify social problems, their severity and interrelatedness to the medical situations, as well as, assessing family strengths and weaknesses and discussing alternate methods of alleviating the situation.
- Assist team members in understanding significant social and emotional factors of the patient & family.
- Ensures all care is provided with respect and dignity for patients, reports all complaints, reports all allegations of abuse, misappropriation of patient property and/or any other corporate compliance items.
- Acts as a mentor to other personnel by participating in orientation and assisting with field training and in-service education to contractors and referral sources
- Teaches the patient, family and/or caregiver regarding the progression of the disease process and self-care techniques, which includes providing counseling support/ instructions as ordered by physician.
Skills
- Must have effective communication, comprehension, documentation, interpersonal and computer skills.
- Ability to assess patient needs and formulate individualized patient care plans to meet those needs.
- Ability to work in a field setting and exhibit ability to make sound judgments.
Qualifications
- Education: Master's degree in social work from a school of social work accredited by the Council on Social Work Education
- Licensure: Valid driver’s license and reliable transportation that is insured in accordance with Ohio requirements. Active Ohio Social Work license.
- Experience: Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness and dying. Hospice or home care experience preferred but not required.

The Hospice Social Worker is responsible for the provision of a wide range of psychosocial services to terminally ill patients and their families.
Shifts Available: PT
Responsibilities:
- Conduct initial comprehensive psychosocial and bereavement risk assessment of patient/family/caregiver in a thorough, accurate, and compassionate fashion within five (5) calendar days after the patient’s election of Hospice care.
- Function as an expert and facilitator at obtaining and making referrals to community resources to meet the needs of patients and families including obtaining financial assistance when indicated and referring patients and families to community agencies with appropriate follow-up.
- Assist family/caregiver in placement of patient in a facility for basic care, respite or inpatient when appropriate as indicated.
- Provide psychosocial and anticipatory grief counseling to patients, families, caregivers and significant others relative to the impact and implications of the terminal disease and patient care.
- Assesses the psychosocial status of patient/family as it relates to capacity to cope with diseases, terminal illness, environment and death. Provides counseling services as applicable and assists in utilizing community resources. Communicates findings to the team.
- Participate in the development of the plan of care and treatment. Holds case conferences as needed.
- Prepare social histories, evaluations and plan interventions based on findings.
- Updates, maintains and submits clinical documentation according to agency guidelines in an accurate and timely manner. Assists with discharge planning.
- Attends and participates in scheduled interdisciplinary team meetings (IDT) to coordinate care plans, follow up on changes, problem solve, identify social problems, their severity and interrelatedness to the medical situations, as well as, assessing family strengths and weaknesses and discussing alternate methods of alleviating the situation.
- Assist team members in understanding significant social and emotional factors of the patient & family.
- Ensures all care is provided with respect and dignity for patients, reports all complaints, reports all allegations of abuse, misappropriation of patient property and/or any other corporate compliance items.
- Acts as a mentor to other personnel by participating in orientation and assisting with field training and in-service education to contractors and referral sources
- Teaches the patient, family and/or caregiver regarding the progression of the disease process and self-care techniques, which includes providing counseling support/ instructions as ordered by physician.
Skills
- Must have effective communication, comprehension, documentation, interpersonal and computer skills.
- Ability to assess patient needs and formulate individualized patient care plans to meet those needs.
- Ability to work in a field setting and exhibit ability to make sound judgments.
Qualifications
- Education: Master's degree in social work from a school of social work accredited by the Council on Social Work Education
- Licensure: Valid driver’s license and reliable transportation that is insured in accordance with Ohio requirements. Active Ohio Social Work license.
- Experience: Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness and dying. Hospice or home care experience preferred but not required.
