• Ongoing: Every other weekend (Sat/Sun, 10:00 AM – 6:00 PM)

Case Overview

We are looking for a reliable and compassionate caregiver to support a kind woman during her bi-weekly home visits with her son. The client normally resides in a group home but returns to her private residence every other weekend to maintain her family bond.

Because the client has a history of a Traumatic Brain Injury (TBI), she requires steady supervision to ensure her safety and well-being.

Key Responsibilities

• Safety & Supervision: Due to impulsivity and forgetfulness from a head injury, the client cannot be left alone.

• Mobility Assistance: Providing "stand-by assist" as the client uses a walker or cane; she is a high fall risk.

• Daily Living Support: Assistance with bathing, dressing, and medication reminders.

• Household Management: Light housekeeping and meal preparation for the client and her son.

• Wound Care Oversight: The client is currently independent with her own wound care (buttocks), but the caregiver should monitor for any changes or needs.

The Environment

• Family Dynamic: You will be working in the home while the client’s son is present. The son is autistic and, while very kind, is not a caregiver; your focus will be entirely on the client’s safety and needs.

• Atmosphere: This is a warm, friendly environment focused on family reunification.

Requirements

• Proven experience in home care or assisted living settings.

• Experience with Brain Injury (TBI) or memory care is a significant plus.

• Patient, observant, and proactive bedside manner.

• Ability to pass a background check and provide references.

🏡 About the Role

Centria caregivers provide essential in-home support to help individuals remain safe, comfortable, and independent. You’ll work one-on-one with clients in a private home setting, supporting their unique care needs.

✅ Responsibilities

• Assist with mobility and transfers to prevent falls
• Provide basic personal care, including hygiene, toileting, and dressing
• Monitor and assist with oxygen therapy as needed
• Prepare light snacks or drinks upon request
• Offer companionship and emotional reassurance throughout the shift
• Assist in positioning and turning to prevent discomfort
• Document and report changes in client condition promptly
• Respond to emergencies following proper procedures
• Perform duties as outlined in the patient’s plan of care
• Monitor client’s physical and mental condition, intake/output, and exercise
• Maintain a safe, clean, and healthy care environment
• Follow professional standards, care policies, and local/state/federal requirements

📝 Qualifications

• CNA, HHA, or equivalent experience in home care preferred
• Experience working with fall-risk and mobility-impaired clients strongly desired
• Must be comfortable assisting with personal care tasks
• Reliable, patient, and attentive to individual needs
• Comfortable working in a private home setting
• Ability to work independently and follow a care plan
• Strong oral and written communication skills
• Ability to follow verbal and written instructions
• Comfortable using tablets/computers for documentation
• Strong attention to detail and ability to follow all care protocols
• Ability to address minor medical concerns as they arise

To be eligible for employment, all caregivers must provide the following:

• Valid Driver’s License
• Current CPR Certification
• Clear Background Check
• Negative TB Test Results
• Proof of Auto Insurance (with your name listed on the policy)
